Services:Coaching, somatic education, sound-based practices, nervous system support, emotional awareness work
1. Scope of Services
I understand that services provided through Nervana are coaching, educational, and somatic-based practices intended to support emotional awareness, nervous system regulation, and personal growth.
These services are not psychotherapy, mental health treatment, medical care, or licensed healthcare services.
No diagnosis, treatment, cure, or prevention of any medical or psychological condition is being provided.
2. Not a Substitute for Medical or Mental Health Care
I understand that Nervana services are not a replacement for medical care, psychiatric care, or therapy.
I am encouraged to seek support from licensed professionals for any medical or mental health concerns.
I affirm that I am responsible for my own physical and emotional well-being during and after sessions.
3. Assumption of Risk
I understand that somatic practices, emotional processing work, breathwork, sound-based practices, and introspective coaching may bring up emotional or physical sensations.
I voluntarily choose to participate and assume full responsibility for any effects that may arise from my participation.
I understand that discomfort, emotional release, or temporary distress may occur as part of inner work.
4. Personal Responsibility
I agree that I am fully responsible for my own choices, actions, and outcomes resulting from participation in Nervana services.
I understand that the practitioner does not guarantee results.
5. Release of Liability
I hereby release, waive, and discharge Nervana / Trish Nesler from any and all liability, claims, demands, or causes of action arising from participation in services, to the fullest extent permitted by law.
I agree not to hold the practitioner responsible for any emotional, psychological, physical, or financial outcomes related to my participation.
6. Confidentiality
All client information is treated as confidential.
However, confidentiality may be broken if:
There is risk of harm to self or others
There is suspected abuse or legal obligation to report
Required by court order or law
7. Payment & Cancellation
Payment is required as agreed upon at time of booking.
Cancellations require at least 24 hours notice unless otherwise stated. Missed sessions or late cancellations may be charged in full.
8. Consent to Services
By signing below, I confirm that:
I am voluntarily choosing to participate
I understand the nature of the services provided
I have read and agree to this waiver and informed consent
I am at least 18 years of age
